What are the various headaches & migraines physiotherapy can treat?​

Types of headaches we can help with

Chronic daily headaches are characterised by persistent and frequent head pain, often occurring for more than 15 days a month. Their causes can vary and may involve tension, stress, or even medication overuse. Those with this condition have extreme difficulty in concentrating due to persistent symptoms.

Cervicogenic headaches originate from issues in the neck or cervical spine and often manifest as pain at the back of the head, radiating to the forehead and temple. This type of headache can be challenging to diagnose as the pain is perceived in the head, but its source is in the neck. These headaches can result from structural problems, such as muscle tension, joint dysfunction, or disc degeneration. 

Cluster headaches are excruciatingly painful and occur in cyclical patterns, often on one side of the head, a rare type of headache. While physiotherapy may not directly treat the underlying cause of cluster headaches, it can play a supportive role in managing associated muscle tension and promoting relaxation. 

Whiplash headaches result from sudden, forceful neck movements, often due to car accidents or trauma. It can occur during a motor vehicle accident whereby the driver has been hit from the front-end, rear-end or a side impact. Whiplash is described as an acceleration and deceleration mechanism of energy transferred into the neck.

Sinus headaches are often accompanied by facial pain and pressure, thought to result from sinus inflammation or congestion. This type of headache is bilateral in nature, where symptoms are felt on both sides of the sinuses/nose.

Cyclical vomiting syndrome involves recurring episodes of severe nausea and vomiting. While not primarily a headache disorder, it can be associated with migraines in some cases. It involves episodes of severe vomiting followed most often by complete symptom-free periods, hence the cyclical nature of the disorder. The episodes can last anywhere from 1 hour up to 10 days. 

Occipital neuralgia leads to sharp, shooting pain along the path of the occipital nerves, which run from the base of the skull to the scalp. It is a medical disorder that presents with piercing, throbbing, or electric-shock-like chronic pain in the upper part of the neck, back of the head and behind the ears. For some people, the pain will present in the forehead, behind the eyes, or in the scalp. Occipital neuralgia is usually unilateral, meaning it is only on one side of the head.

Trigeminal neuralgia involves severe facial pain due to the dysfunction of the trigeminal nerve. This nerve carries sensation from your face and scalp to your brain.

Types of migraines we can help with ​

Chronic migraines involve experiencing headaches on more than 15 days per month for at least three months. Migraine headaches are a type of headache that typically involves intense, throbbing pain, often on one side of the head. They can be accompanied by other symptoms like nausea, vomiting, sensitivity to light and sound, and visual disturbances.

Menstrual migraines occur around the menstrual cycle due to hormonal fluctuations. They are triggered by drops in estrogen levels. Menstrual migraine affects women at different age group throughout their menstrual cycle. Their migraine can typically peak on the day of their cycle, a few days before, or a few days after their cycle and can last for a couple of hours to 5 days (most commonly 3 days).

Migraine with aura: 

A migraine with aura is a distinctive form of migraine characterized by moderate to severe throbbing head pain on one or both sides of the head, akin to the more common migraine without aura. Alongside this pain, individuals with migraine with aura experience transient and reversible neurological symptoms known as “auras.” These auras can encompass a range of sensory changes, including visual disturbances like flashing lights and zigzag lines, as well as alterations in sensation such as tingling and numbness. Additionally, some individuals may encounter temporary language difficulties during the aura phase.

Migraine without aura: 

This is the most prevalent type of migraine. It involves moderate to severe throbbing head pain on one or both sides of the head, often accompanied by symptoms such as nausea, vomiting, sensitivity to light and sensitivity to sound. It does not have an aura phase.

Vestibular migraines are characterised by dizziness, vertigo (a sensation of spinning or whirling), and imbalance, often accompanied by headache symptoms. 

Hemiplegic migraines are rare and involve temporary paralysis or weakness on one side of the body, often accompanied by other migraine symptoms. In addition to weakness, individuals with hemiplegic migraines might experience other neurological symptoms like confusion, difficulty speaking (aphasia), problems with coordination, and changes in consciousness.
